Elsie Allen High School Heart Screening Volunteer Sign up form

On behalf of the Kyle J. Taylor Foundation and Healthy Petaluma District & Foundation, thank you for volunteering for our Free Youth Heart Screening at Elsie Allen High School on Sunday, Feburary 23, 2025.  The screening is completely free for anyone aged 12-25.  We will be open from 9am to 4pm and will close to provide lunch for our volunteers from 12pm to 1pm.  Please complete your contact information and volunteer information, and you will select one volunteer shift.  If you have questions or need to make changes or cancellations please reach out to our Volunteer Coordinators at volunteers@kylejtaylor.org
