Folsom High School Free Youth Heart Screening

Registration is currently closed for this event.  We will be accepting walk-ins at the event 9am-12pm and 1:00pm-3:30pm

On behalf of the Kyle J. Taylor Foundation, UC Davis Health and Project ADAM Sacramento, thank you for registering for our Free Youth Heart Screening at Folsom High School on Sunday, April 14th.  The screening is completely free for anyone aged 12-25.  We will be open from 9am to 4pm and will close to provide lunch for our volunteers from 12pm to 1pm.  Please complete your contact information and participant(s) information below, and you will select your 30-minute appointment check-in window(s) on the next page.  If you have questions or need to make changes or cancellations please reach out to Jennifer Sarmento at jennifer@kylejtaylor.org or 408-893-4233
